Please click the link below to download the Town of Davie Volunteer Application form. Once downloaded, please fill out all pages and turn it into the Davie PAL staff ASAP.
- Pages 3 must be signed in the presence of a Notary. Please have documents notarized PRIOR to scheduling your fingerprint appointment.
- ALL Broward / Dade certified referees must complete this process and and submit completed/notarized forms to the Davie PAL. Please include a copy of your Broward and/or Dade County certified Referree badge and avoid $35 application fee. If you are not a certified Referree, you will have to pay the $35 application fee.
In order for me to utilize the Davie PAL to run the basketball league, The Davie PAL requiring this of all of my volunteer coaches and certified (Dade/Broward) officials. All South Florida Youth Basketball volunteers of the Davie location and over the age of 18 must be fingerprinted.
You will be required to pay a $35 (cash only) fee to the Davie PAL
