League Experience : Has your child played with our league before? If so, how many sessions did they play in?

Shirt Size : Please be sure this is correct. If incorrect, the cost of a second shirt shall be your responsibility.

Practice Night Exclusion : If applicable, please select the night that your child cannot attend practice.
