Please click the link below to download the Town of Davie Volunteer Application form. Once downloaded, please fill out all pages.
Pages 3 must be signed signed by a witness.
All South Florida Youth Basketball coach’s volunteering out of the Davie location and over the age of 18 must be fingerprinted.
You will be required to pay a $35 (cash only) fee to the Davie PAL
